This application will place your student on the waitlist for the lottery for the next school year. Fill out this form by April 1st at 5 pm. You will need to fill out an application FOR EVERY STUDENT you would like to enroll. Families will be notified by April 15th that the lottery was held and will be given any additional information.

Pursuant to statute, Fossil Distance Learning Program is a public school and does not have to accept students who have been expelled from another Oregon public school. Students who have been expelled from an Oregon public school and submit an enrollment application to FDLP will be considered on an individual basis. Parents are responsible for informing FDLP if their students have been expelled from school upon initial submission of the enrollment application.
